1)- CHALLENGER DISASTER (JANUARY 28, 1986)
The Challenger space shuttle, programmed by Nasa in 1986, exploded 73 seconds after taking off. 7 researcher astronauts have died. The cause of the accident was that the shuttle leaked fuel.
2)- SOYUZ 1 (April 23, 1967)
The Soviets, who were in a space race with the US, launched Soyuz 1 into space, although it was known to be dangerous. The cosmonaut of the flight, Vladimir Komarov, crashed to the ground when the parachute system malfunctioned while returning to Earth.
3)- COLUMBIA SPACE SHUTTLE ACCIDENT (FEBRUARY 1, 2003)
The space shuttle STS-107 was shattered 16 minutes before landing on Earth. The cause of the accident, which resulted in the death of 7 astronauts, was the damage caused by a fragment from the main fuel tank during take-off to the left wing of the shuttle.
4)- SOYUZ 11 (June 30, 1971)
During the landing of the Soyuz 11 space shuttle, a faulty vent was opened, and caused to cabin pressure drop. 3 astronauts who were not wearing spacesuit because they were on the process of landing have drowned and died.
5)- APOLLO 1 (January 27, 1967)
The Apollo program, the first US crewed mission, resulted in the death of three astronauts on board as a result of a fire in the cabin during the launch rehearsal on 27 January.
6)- APOLLO 12 (November 14, 1969)
Apollo 12 encountered two lightning strikes during its take-off that caused various malfunctions. The first hit occurred 36 seconds after takeoff and disabled the three fuel cells, and the second hit occurred 52 seconds after takeoff and disabled the onboard steering platform.
7)- VSS ENTERPRISE CRASH (Oct. 31, 2014)
The two piloted spacecraft SpaceShipTwo disintegrated 10 seconds after takeoff during its test flight. One of the pilots has died while the other was seriously injured.
8)- APOLLO-SOYUZ TEST PROJECT (July 15, 1975)
A malfunction in the altitude-controlling reaction control system RCS caused toxic nitrogen tetroxide to enter the cabin of American Apollo astronauts. Fortunately, when the spacecraft landed, the cabin was ventilated and none of the astronauts were fatally injured.
9)- STS-9 (December 8, 1983)
A fire started on the STS-9 spaceship, two minutes before the landing, which no one noticed, when two of the three APUs burned in the APU compartment behind the shuttle. Although the fire did not affect the landing, it did great damage to the shuttle.
10)- LUCA PARMITANO (July 16, 2013)
As Luca Parmitano was working outside the ISS, he felt the water start to fill into his space suit. In the non-gravity environment, the water filled directly into the astronaut’s helmet. Returning to the station in a short time, Parmitano narrowly avoided drowning in space.
